            -  Georgia State Immunization Form (3231)
            -  Georgia State Hearing, Vision & Dental Screening Form (3300)
            -  Child's Original Social Security Card AND
            -  Two Current Proofs of Residence:
                1. one utility bill (gas, electric or water) AND 
                2. home contract or leasing agreement​

PARKING PERMITS AND LOCKER RENTALS CAN BE PAID FOR ONLINE AT THE FOLLOWING LINK - https://osp.osmsinc.com/fultonga/

  • On the left side of screen under schools, select “High”
  • Locate “Northview HS” from the categories on the left side of the page
  • Select the activity
    • Locker Rental $15
    • Parking Permit $100
  • Click “Add To Cart”
  • Click “Checkout” to complete your payment
  • Login in if a returning user or create an account for a new user
  • Assign a student to your activity (need student ID number which you may get from HAC). If you have more than one student, you will need to do this step for each student – one student per transaction.
  • If your student has been added to system, select their name from the Select Student Profile List box. Otherwise, click on Add Student Profile button. Add a new student, enter their information and press save changes button.
  • OSP returns to the assign student screen where you can now select the student and assign to the activity and press next button. If multiple students, do for each student
  • Next enter your address and payment information - press next button to proceed
  • Enter your card button
  • Click review order button
  • Click place order button
 
Once transaction is complete, OSP will email you a thank you message and the order number. You must bring your printed receipt to Information Day/Freshmen Orientation to prove you have purchased these items.
 
Other items on the checklist must be purchased individually.  Payments may be in cash or check.

SENIOR PORTRAITS FOR YEARBOOK  need to be taken by Cady Studios no later than August 30, 2019. If you have not had your senior portrait taken, make an appointment to do so immediately.  To make an appointment go to http://session.cadystudios.com/ or call Cady Studios at 678-722-3449.
